The amounts in parentheses are for established patients, non parentheses are for new patients.
($70) $90 One Issue, Quick Visit: 15 Minutes
Examples include: contraceptive counseling and prescription; upper respiratory infections; one chronic health condition and out of medication issue; counseling regarding child vaccination schedule; consultations and “meet and greets.”
($130) $160 Head to Toe Physical Exam For Adults and Children (Sports Physical, Well Child Check): 30 Minutes
Head to Toe exams include an annual Pap smear and breast exam for women and a prostate exam for men. A children’s exam is often called a Well Child Check. Provider may recommend tests and/or immunizations. Laboratory costs are not included in the exam costs.
($70) $90 Urgent Injuries to Joints, Bones and Muscles:
15 Minutes plus Interpreting Diagnostic Images
Common injuries include strains, sprains, and fractures. X-rays are not included with the price of exam, but may be recommended.
($150) $180 Procedure Visit (Includes All Initial Supplies)
Laceration (bad cut) Repair, Abscess Drainage, Cyst Removal, Wart Removal, Foreign Body Removal, Skin Biopsy (lab costs not included), Trigger Point Injection (medication included), Depo Shot (includes pregnancy test and medication), B-12 shot (medication included).
($120) $170 Multiple Issues Management: 20-30 Minutes
Needs medical management of more than one chronic health condition. Examples include: high blood pressure, high cholesterol and out of medications; diabetes with uncontrolled blood sugars, needs medication changes and has a rash

*Lab tests and blood draws are only available as part of a provider’s visit, or as a follow-up appointment. These labs are available without a venous blood draw.
$25 Draw Fee
This fee covers the time and equipment required for more complicated blood tests that will be sent to an outside lab. Examples include Cholesterol screening, Thyroid check, and many labs required for chronic health conditions and medication management.
The external lab will bill you directly. We can provide a price estimate of the lab charges. We do not regulate their prices however we have advocated for our patients and chosen labs with competitive pricing.
$30/5 Minutes: Additional Health Counseling/Advising Time
The choice is yours. If you have a concern, we can help you address it, provide referrals, community resources and answer your questions. Sometimes 15 minutes is simply not enough time. Again, the choice is yours.

$55 Telephone Consult: 5-15 Minutes
You can schedule a telephone consult appointment. Have a question about a symptom? Need health advice? Need triage* on the weekend or in the evening? Not sure you should go to the Emergency Room or make an office visit?
*We cannot examine you over the phone, but we can ask specific questions and make a determination whether you can manage your condition at home, need an office visit or should go to urgent care.
